Run it while they are away: Execute Rotate while the target steps away from their desk, so they come back to a rotated desktop with a question mark cursor
Create a disguised shortcut: Name the shortcut something like "Budget Report" or "Email" and place it on their desktop
Startup folder: Add it to the target's Startup folder (Win+R, type shell:startup) so it runs automatically at login
Run it multiple times: Since each run produces a random result, run it several times in a row for increasingly chaotic effects
