Will the victim know it is a prank right away?

Not usually. Random mouse clicks and movements look like legitimate computer problems. The victim will think they have a hardware issue, a virus, or a rogue driver. They will try unplugging their mouse, running antivirus scans, and checking for software conflicts. The more realistic your preset or custom configuration, the longer the prank can go undetected.
